I fully understand that there are risks, hazards, and dangers associated with the services and activities provided by Geneva Hills. These risks include the uncertainties of the weather, vehicles on park property, activities and amenities, other visitors and their pets, and with the uncertainty of conditions in an outdoor environment. I am voluntarily participating in these activities and am using the services of Geneva Hills with full knowledge of the risks involved and I accept and assume all risk of the activities and services whether or not described above, known or unknown, foreseeable or unforeseen. I understand that these risks may include loss of property, injury to person, or death. I agree for myself and for all minors for whom I am parent or guardian for my heirs, successors and assigns, that I release Geneva Hills, its owners, agents and employees from any and all claims, damages, and injuries, including all injuries to person or to property, arising directly or indirectly out of the use of lake, pool, archery tag, laser tag, hiking, camping, boating, fishing, hatchet throwing, or any other program or activity provided by Geneva Hills. This release and the following indemnification includes any claims arising, in whole or in part from negligent acts or omissions of Geneva Hills, its owners, agents or employees. I understand that the description of these risks is not complete and that other unknown or unanticipated risks may result in injury or death. I agree for myself and for all minors for whom I am parent or guardian, for my heirs, successors, and assigns, that I shall defend, indemnify and hold harmless Geneva Hills, its owners, agents, and employees from all losses, claims, expenses and demand, including attorney fees, that may be incurred by Geneva Hills, its owners, agents and employees, that are related, directly or indirectly, to my or any minor from whom I am parent or guardian, us or participation in the services or activities and/or amenities provided by Geneva Hills. I grant to Geneva Hills and their legal representatives and assigns the irrevocable and unrestricted right to use and publish photographs of me, or in which I may be included, for editorial, trade, advertising and any other purpose and in any manner and medium; and to alter and composite the same without restriction and without my inspection or approval. I hereby release Geneva Hills and their legal representatives and assigns from all claims and liability relating to said photographs. I have read and fully understand all questions requested in this application. I certify that all of the answers given by me are true, accurate and complete and understand that the completion and/or execution of this application does not insure me a position, nor does it obligate me or the organization in any way. I understand that the omission and/or misrepresentation of facts requested is just cause for immediate dismissal without prior notice. I also give authorization to obtain character references from those I have listed, give consent for a background check of criminal convictions, and give consent for a background check of child abuse or child neglect. In the event of a medical emergency, I give permission to the physician selected by the Camp Director to hospitalize, secure proper treatment for, and to order injections, anesthesia or surgery. I also give my consent to routine, non-surgical medical care. If accepted for service, I understand I will not receive any wages or compensation for any of my services. I agree to abide by all the policies, rules, regulations and standards of Geneva Hills Group Inc.
